Wesley’s Settlement Services provide client-centred, culturally responsive support to newcomers—including Government-Assisted Refugees (GARs), non-GAR newcomers, and persons in need of protection—who face complex and ongoing barriers to settlement and integration. Through our Intensive Case Management (ICM) approach, Wesley delivers personalized, outcome-focused support in Hamilton and Brantford, helping individuals and families build stability, independence, and well-being.
Services are tailored to each client through initial, ongoing, and exit assessments, and are delivered in the community, in homes, and through flexible in-person and digital supports. Clients work with dedicated Intensive Case Managers, who are qualified Social Workers, to navigate essential systems such as housing, healthcare, education, income supports, and community services. The program includes enhanced housing maintenance support, community accompaniment, and coordinated referrals to reduce service gaps and duplication.
Wesley’s case management model is delivered through five core service pillars:

A dedicated Health and Well-Being Navigator provides short-term counselling, wellness education, and referrals to specialized mental health services, supporting clients’ emotional well-being alongside their settlement goals.
As the sole provider of case management services for newcomers in Hamilton and Brantford, Wesley ensures a coordinated, consistent, and high-quality response that empowers newcomers—individuals, families, children, youth, 2SLGBTQI+ clients, and persons with disabilities—to overcome barriers and build strong foundations for successful integration in Canada.
The Seniors Outreach supports vulnerable newcomer adults and seniors (55+) across the Greater Hamilton Area through a combination of remote and in-person case management at multiple outreach locations.
Our mobile outreach team helps seniors facing complex physical or mental health challenges, isolation, and difficulty coping– particularly those without a strong social support network.
The program promotes wellness, belonging, and improved quality of life, empowering clients to navigate challenges and stay engaged in their communities.
