Youth Outreach Workers – Brantford
Mobile workers engaging at-risk youth aged 12-21 years and their families, to better navigate and connect with services and supports in their communities to improve youth outcomes.
Provincial Youth Outreach Workers build relationships with youth and their families, in order to help them identify and articulate their needs
The program focuses on at-risk youth who live in identified priority communities and/or belong to a distinct priority population. These youth may experience increased barriers in accessing the opportunities that can help them to develop their capacity to make healthy life choices and achieve their goals.
