Seniors Outreach Program
Delivered via remote and in-person case management and situated in multiple outreach locations across the greater Hamilton area, the mobile Seniors Outreach Program (SOP) promotes positive wellness and belonging and enhances the ability of clients to enjoy life and deal with challenges. The outreach team provides services to a targeted newcomer population: vulnerable newcomer adults and seniors (aged 55 and older) who may experience complex physical and/or mental health problems, isolation, and coping difficulty & who don’t have access to a social support network.
